The three paths

Aux trois sentiers is breaking new ground to address a fundamental need of an aging population. Through three complementary programs, the organization provides a continuum of specialized end-of-life care while offering patients an alternative to hospitalization.

1. Specialized care provided by a multidisciplinary team for patients who wish to die at home, covering all care related to the terminal phase (the last six weeks of life), including specialized care currently available only in certain hospital settings. Rapid Response Unit

Implementation sequence

Paths 1 and 2 are currently being implemented as part of a pilot project in partnership with the CHUM and the CIUSSS West-Central Montreal / Jewish General Hospital

Path 1 – The Rapid Response Unit (RRU) brings together a team of compassionate caregivers who provide all necessary end-of-life care at home for those wishing to avoid hospitalization.

Path 2 – A continuum of care and support for patients and caregivers, offering respite and complementary care to ease this life transition. Learn more

Path 3 – The envisioned opening of a Home that will accompany people through the final stage of life, offering short stays for symptom stabilization during the palliative phase and much-needed respite for their loved ones. It will also serve as a center of expertise and learning, dedicated to training and supporting healthcare professionals.
