About Aux trois sentiers

Aux trois sentiers is a non-profit organization whose mission is to provide a continuum of personalized home-based care and services to individuals at the end of life and to their loved ones.

Our services are organized around three complementary pathways:

Path 1 | Rapid Response Unit (RRU)
A specialized team providing an alternative to hospitalization by delivering intensive end-of-life care at home during the last four to six weeks of life.

Path 2 | Support for Family Caregivers
Programs and services designed to support loved ones throughout the end-of-life journey and beyond.

Path 3 | End-of-life Home and Learning Centre (planned for 2029–2030)
A future palliative care home and centre of excellence dedicated to clinical practice, education, and innovation in palliative and end-of-life care.

At Aux trois sentiers, we envision an end-of-life experience rooted in humanity and compassion, while offering meaningful alternatives to hospitalization. To bring this vision to life, we are seeking dedicated professionals who are passionate about improving access to end-of-life care and making a tangible difference in the lives of patients and families.
We are looking for individuals who are inspired by innovation, energized by meaningful challenges, and eager to help build a transformative model of care alongside colleagues who share a deep commitment to caring for people at the end of life and those who love them.
